Next Steps for Alumni
Items Needed
Now that you have been accepted for distance career services, you need to complete and mail the following documents:

	Professional Disclosure and Consent Form - Word version or .pdf version.
	Please print, read and sign the form.
	Proof that you are a Rutgers - New Brunswick alumnus or alumna.
	Accepted proof: Copy of diploma, alumni association card, student ID card, or unoffical transcript.
	A check for $50 payable to Rutgers, The State University.
	Mail all items to: Chrystal McArthur Career Services Rutgers University 46 College Avenue New Brunswick, NJ 08901-8541

Cancellation Policy
	All cancellations must be received 48 hours prior to the scheduled appointment.
	Call 732-932-7997 or email Chrystal McArthur at careerservices46@careers.rutgers.edu

Refund Policy
	If a cancellation is received 48 hours prior to the scheduled appointment a refund check will be mailed to your address.
